ABSTRACT

Abstract Objective To evaluate the accuracy and patient acceptability toward self-sampling using a new device - SelfCervix® - for detecting HPV-DNA. Methods A total of 73 women aged 25-65 who underwent regular cervical cancer screening from March to October 2016 were included. Women performed self-sampling followed by a physician-sampling, and the samples were analyzed for HPV-DNA. After that, patients were surveyed about their acceptability of self-sampling. Results HPV-DNA detection rate of self-sampling presented high accuracy and was similar to physician-collection. Sixty-four (87.7%) patients answered the acceptability survey. Most patients (89%) considered the self-sampling comfortable, and 82.5% preferred self-sampling to physician-sampling. The reasons cited were time-saving and convenience. Fifty-one (79.7%) reported that they would recommend self-sampling. Conclusion Self-sampling using the new Brazilian device SelfCervix® is not inferior in HPV-DNA detection rate compared with physician-collection, and patients are supportive of the method. Therefore, it might be an option to reach under-screened populations in Brazil.


Resumo Objetivo Avaliar a acurácia e aceitabilidade da auto-coleta utilizando um novo coletor - SelfCervix® - para a detecção de DNA de HPV. Métodos Foram incluídas no estudo 73 mulheres com idade entre 25-65 anos que realizaram seu rastreamento regular do câncer de colo do útero entre Março e Outubro de 2016. Estas mulheres realizaram a auto-coleta, seguida de coleta profissional e as amostras foram analisadas paraa presença de DNA de HPV. Após, elas responderam um questionário sobre a experiência da auto-coleta. Resultados As taxas de detecção de DNA de HPV por auto-coleta foram altas e similares as da coleta profissional. Sessenta e quatro (87,7%) pacientes responderam o questionário de experiência. A maioria (89%) considerou a auto-coleta confortável, e 82,5% preferiram o método comparado a coleta profissional. As razões citadas foram economia de tempo e conveniência. Cinquenta e uma (79,7%) mulheres confirmaram que recomendariam a auto-coleta. Conclusão Auto-coleta utilizando o novo coletor desenvolvido no Brasil não é inferior na detecção de DNA de HPV quando comparada a coleta profissional, e apresenta uma boa aceitabilidade pelas mulheres. Desta maneira, pode ser uma opção para alcançar populações que não realizam o rastreamento padrão.

ABSTRACT

Scoliosis is one of the common diseases in adolescents. It can rapidly progress during the peak growth period. Scoliosis school screening (SSS) is the main way for early detection and treatment of this disease. The SSS project began in the 1960s and was gradually promoted from the United States to the world. However, some problems were gradually exposed during the implementation of the project, such as the high false positive rate of screening methods, potential radiation damage, the uncertainty of the potency ratio of screening and the lack of evidence-based medical evidence of the effectiveness of high-level conservative treatment, which led to many European and American countries stopping the implementation of SSS. However, with the progress of research related to diagnosis and treatment of scoliosis, based on the latest evidence-based medical evidence assessment, the United States Preventive Medicine Task Force again adjusted the recommendation level of SSS to "no recommendation, no objection" in 2018. In recent years, SSS has gradually received extensive attention from the Chinese government and society. Five national ministries and commissions also issued a document in 2021 to include scoliosis in the monitoring of common diseases among students. However, the implementation of the project should also refer to the effectiveness criteria of disease screening recommended by the World Health Organization. In the future, with the improvement of the accuracy of scoliosis screening methods, the development of multi-mode screening such as artificial intelligence, the emergence of non-radiation detection technology and the improvement of the effectiveness of conservative treatment of mild and moderate scoliosis, the long-term and large-scale implementation of SSS project and the early prevention and control of scoliosis will be possible to truly achieve.

Introducción. Las pruebas diagnósticas de tamizaje son aquellas pruebas que son capaces de identificar un factor de riesgo o mutaciones genéticas que predicen el inicio ulterior de la enfermedad, así como también las pruebas que ponen de manifiesto alteraciones estructurales de la enfermedad antes que la enfermedad progrese y se vuelva sintomática. Métodos. Se hizo una revisión de la literatura para establecer los fundamentos teóricos científicos que sustentan a las pruebas de diagnóstico de tamizaje y las condiciones y requisitos que se deben cumplir para introducirlas en el ámbito clínico o como programas de salud pública. Resultados. Se estableció la diferencia conceptual entre la detección precoz y el diagnóstico temprano y la diferencia entre tamizaje de prevalencia y tamizaje de incidencia. Se dieron a conocer las indicaciones y criterios científicos para la realización de las pruebas de tamizaje. Se puntualizó la importancia de la duración del tiempo de adelanto en la eficacia de las pruebas diagnósticas de detección precoz. Se argumentaron las razones por las cuales era necesario la realización de experimentos clínicos aleatorizados para evaluar la eficacia de las pruebas diagnósticas de detección precoz en la prevención secundaria de la enfermedad. Conclusiones. Las pruebas diagnósticas de tamizaje hacen posible la introducción de intervenciones en el ámbito de la prevención primaria, como también en el escenario de la prevención secundaria de las enfermedades.


Introduction. Screening diagnostic tests are those tests that help to identify a risk factor or genetic mutations that predict the subsequent onset of the disease, as well as tests that reveal structural alterations of the disease before the disease progresses and becomes symptomatic. Methods. A literature review was performed to establish the scientific theoretical fundamentals that support diagnostic screening tests and the conditions and requirements that must be met to introduce them in the clinical setting or as public health programs. Results. The conceptual difference between early detection and early diagnosis and the difference between prevalence screening and incidence screening was established. Indications and scientific criteria for conducting screening tests were presented. The importance of the duration of the lead time in the efficacy of early detection diagnostic tests was pointed out. The reasons why it was necessary to carry out a randomized clinical experiment to evaluate the efficacy of early detection diagnostic tests for early diagnosis in the secondary prevention of the disease were confronted. Conclusions. Screening diagnostic tests make it possible to introduce interventions in the field of primary prevention, as well as in the setting of secondary prevention of diseases.

ABSTRACT Objective: To describe the performance of a pulmonologist-led lung cancer screening program using low dose CT (LDCT) in a cohort of outpatients with stable respiratory diseases in the Brazilian public health care system. Methods: This was a retrospective analysis of the first two rounds of lung cancer screening of patients enrolled in the program. Inclusion criteria were being between 55 and 80 years of age, being a current or former smoker (smoking cessation ≤ 15 years), and having a smoking history ≥ 30 pack-years. LDCT results were interpreted in accordance with the Lung CT Screening Reporting and Data System, and those with a score of 3 or 4 were considered positive screening. Incidental pleuropulmonary findings were sought in all reports. Results: LDCTs were requested for 791 patients during the study period, and 712 patients (90%) met the screening criteria. The mean patient age was 63 years, and most participants were current smokers (56%) with emphysema (78.5%) and other pleuropulmonary findings on CT (64%). Screening was positive in 14.0% and 5.6% of the cases in the first and second screening rounds, respectively. Lung cancer was detected in 1.5% of the patients in both first and second rounds (positive predictive value: 11.0% and 26.6%, respectively). The rate of early-stage (TNM I or II) screen-detected non-small cell carcinoma was 64.3%. Of the patients with positive screening, 19% were lost to follow-up before investigation was complete. Conclusions: The results of this screening program suggest its adequate performance in a cohort of patients with significant respiratory morbidity. The loss to follow-up rate highlights the need for constant monitoring and interventions to ensure adherence.


RESUMO Objetivo: Descrever o desempenho de um programa de rastreamento de câncer de pulmão conduzido por pneumologistas usando TC de baixa dose (TCBD) em uma coorte de pacientes ambulatoriais com doença respiratória estável no sistema público de saúde brasileiro. Métodos: Análise retrospectiva das duas primeiras rodadas de rastreamento de câncer de pulmão em pacientes inscritos no programa. Os critérios de inclusão foram ter idade entre 55 e 80 anos, ser fumante atual ou ex-tabagista (cessação do tabagismo ≤ 15 anos) e carga tabágica ≥ 30 anos-maço. Os resultados do TCBD foram interpretados de acordo com o Lung CT Screening Reporting and Data System, e aqueles com pontuação 3 ou 4 foram considerados exames positivos. Achados pleuropulmonares incidentais foram verificados em todos os relatórios. Resultados: TCBD foram solicitadas para 791 pacientes durante o período do estudo, e 712 pacientes (90%) preencheram os critérios de rastreamento. A média de idade dos pacientes foi de 63 anos, e a maioria dos participantes era fumante atual (56%) com enfisema (78,5%) e outros achados pleuropulmonares na TC (64%). O rastreamento foi positivo em 14,0% e 5,6% dos casos na primeira e segunda rodada, respectivamente. O câncer de pulmão foi detectado em 1,5% dos pacientes tanto na primeira quanto na segunda rodada (valor preditivo positivo: 11,0% e 26,6%, respectivamente). A taxa de carcinoma de células não pequenas detectado em estágio inicial (TNM I ou II) foi de 64,3%. Dos pacientes com rastreamento positivo, 19% foram perdidos no seguimento antes da conclusão da investigação. Conclusões: Os resultados deste programa de rastreamento sugerem um desempenho adequado em uma coorte de pacientes com morbidades respiratórias significativas. A taxa de perda de seguimento destaca a necessidade de monitoramento constante e intervenções para garantir a adesão.

ABSTRACT Hypertension remains the leading cause of cardiovascular disease globally despite the availability of safe and effective treatments. Unfortunately, many barriers exist to controlling hypertension, including a lack of effective screening and awareness, an inability to access treatment and challenges with its management when it is treated. Addressing these barriers is complex and requires engaging in a systematic and sustained approach across communities over time. This analysis aims to describe the key elements needed to create an effective delivery system for hypertension control. A successful system requires political will and supportive leadership at all levels of an organization, including at the point of care delivery (office or clinic), in the health care system, and at regional, state and national levels. Effective screening and outreach systems are necessary to identify individuals not previously diagnosed with hypertension, and a system for follow up and tracking is needed after people are diagnosed. Implementing simple protocols for treating hypertension can reduce confusion among providers and increase treatment efficiency. Ensuring easy access to safe, effective and affordable medications can increase blood pressure control and potentially decrease health care system costs. Task-sharing among members of the health care team can expand the services that are delivered. Finally, monitoring of and reporting on the performance of the health care team are needed to learn from those who are doing well, disseminate ideas to those in need of improvement and identify individual patients who need outreach or additional care. Successful large-scale hypertension programs in different settings share many of these key elements and serve as examples to improve systems of hypertension care delivery throughout the world.


RESUMEN A pesar de la disponibilidad de tratamientos seguros y efectivos, la hipertensión sigue siendo la principal causa de enfermedad cardiovascular a nivel mundial. Lamentablemente, el control de la hipertensión enfrenta muchos obstáculos, como la falta de detección y concientización efectivas, la incapacidad de acceder al tratamiento y los retos que plantea su manejo cuando se recibe tratamiento. Resulta complejo abordar estos obstáculos; se debe adoptar un enfoque sistemático y sostenido en todas las comunidades a lo largo del tiempo. El presente análisis tiene como objetivo describir los elementos clave necesarios para crear un sistema de atención eficaz para el control de la hipertensión. Un sistema adecuado precisa voluntad política y un liderazgo que brinde apoyo en todos los niveles de una organización, incluido el punto donde se presta la atención (consultorio o clínica), el sistema de atención de salud y a nivel regional, estatal y nacional. Se necesitan sistemas efectivos de detección y de alcance comunitario para encontrar a las personas sin diagnóstico previo de hipertensión, así como un sistema para el seguimiento y la localización una vez han sido diagnosticadas. La aplicación de protocolos simples para tratar la hipertensión puede reducir la confusión entre los proveedores y aumentar la eficiencia del tratamiento. Garantizar un acceso fácil a medicamentos seguros, efectivos y asequibles puede aumentar el control de la presión arterial y potencialmente disminuir los costos para el sistema de atención de salud. La distribución de tareas entre los miembros del equipo de atención de salud permite expandir los servicios que se prestan. Finalmente, es necesario dar seguimiento al desempeño del equipo de atención de salud y presentar información al respecto, a fin de aprender de las buenas prácticas, difundir ideas entre quienes necesitan mejorar y determinar a cuáles pacientes se debe llegar y cuáles necesitan atención adicional. Los programas de hipertensión a gran escala con buenos resultados en diferentes entornos comparten muchos de estos elementos clave y sirven como ejemplos para mejorar la atención de la hipertensión en todo el mundo.


RESUMO A hipertensão continua sendo a principal causa de doenças cardiovasculares no mundo, apesar da disponibilidade de tratamentos seguros e eficazes. Infelizmente, existem muitas barreiras para o controle da hipertensão, incluindo a falta de rastreamento e a conscientização eficazes, a incapacidade de acesso ao tratamento e desafios de conduta clínica quando ela é tratada. A abordagem dessas barreiras é complexa e requer um enfoque sistemático e sustentado em diferentes comunidades ao longo do tempo. Esta análise tem como objetivo descrever os elementos-chave necessários para criar um sistema eficaz de prestação de intervenções para o controle da hipertensão. Um sistema de sucesso requer vontade política e liderança que o apoie em todos os níveis de uma organização, inclusive no ponto da prestação de cuidados (consultório ou clínica), no sistema de saúde e nos níveis regional, estadual e nacional. Sistemas eficazes de triagem e acolhida são necessários para identificar indivíduos com hipertensão não diagnosticada, e um sistema de acompanhamento e rastreamento após o diagnóstico se faz igualmente necessário. A implementação de protocolos simples para o tratamento da hipertensão pode reduzir a confusão entre os profissionais de saúde e aumentar a eficiência do tratamento. Garantir o fácil acesso a medicamentos seguros, eficazes e acessíveis pode aumentar o controle da pressão arterial e potencialmente diminuir os custos do sistema de saúde. A divisão de tarefas entre os integrantes da equipe de saúde pode expandir os serviços prestados. Finalmente, é necessário monitorar e informar acerca do desempenho da equipe de saúde para aprender com aqueles que estão indo bem, disseminar ideias para aqueles que precisam melhorar e identificar pacientes individuais que precisam de ajuda ou cuidados adicionais. Programas bem-sucedidos de controle da hipertensão em larga escala em diferentes ambientes compartilham muitos desses elementos-chave e servem como exemplos para melhorar os sistemas de atenção à hipertensão em todo o mundo.

Introducción. La diabetes mellitus (DM) es una de las enfermedades crónicas más comunes, siendo una de las causas principales de mortalidad de la población mun-dial. La dermatoglifia es empleada como instrumento para el hallazgo de ciertos aspectos biológicos en diferentes poblaciones. Objetivo. Reconocer, a partir de una revisión sistemática, la relevancia que tiene la dermatoglifia como medio de hallazgo de diabetes mellitus. Metodología. Revisión de literatura científica en bases de datos como Sciencedi-rect, PubMed, Scopus y BVS, en las cuales se extrajo información después de co-rrer la ecuación de búsqueda con términos MESH. Posteriormente, fueron revisados para incluir aquellos artículos relacionados con DM y dermatoglifia. Para todo el proceso se siguió la metodología PRISMA, evaluando los artículos con la escala de sesgo de Cochrane y el nivel de evidencia y recomendación con escala SIGN. Resultados. Después de una revisión de 475 artículos, se incluyeron ocho artículos, y al ser evaluados fueron clasificados cinco artículos 2+ y tres 2-, con nivel de reco-mendación D. Conclusión. La dermatoglifía es una herramienta útil como medio de detección de la DM. Sin embargo, es necesario realizar estudios de cohortes para demostrarlo en di-ferentes poblaciones, como la colombiana. En ese sentido, se encuentra que el 70% de los artículos revisados demuestran que la predicción de hallazgo de DM es efectiva. Sin embargo, el 30% de las investigaciones dentro de esta revisión no consideran que sea una herramienta lo suficientemente óptima para descubrir la patología en la población.


Introduction. Diabetes mellitus (DM) is one of the most common chronic diseases, be-ing one of the main causes of mortality in the world population. Dermatoglyphics is used as an instrument for the discovery of certain biological aspects in different populations.Objective. To recognize, by doing a systematic review, the relevance of dermato-glyphics as a means of finding diabetes mellitus. Methodology. Review of scientific literature in databases such as Sciencedirect, PubMed, Scopus, and VHL, in which information was extracted after running the search equation with MESH terms. After that, they were revised to include those articles related to DM and dermatoglyphics. For the entire process, the PRISMA methodology was followed, evaluating the articles with the Cochrane bias scale and the level of evidence and recommendation with the SIGN scale. Results. After a review of 475 articles, eight articles were included, and when evalu-ated, five articles were classified as 2+ and three as 2-, with a level D recommendation. Conclusion. Dermatoglyphics is a useful tool as a means of detecting DM. However, it is necessary to carry out cohort studies to demonstrate this in different populations, such as the Colombian population. 70% of the reviewed articles show that the prediction of DM finding is effective. Nevertheless, 30% of the investigations within this review do not consider it to be a sufficiently optimal tool to discover the pathology in the population

ABSTRACT

ABSTRACT Lung cancer screening (LCS) programs are increasing worldwide. Incidental findings (IFs) on LCS are defined as low-dose CT findings unrelated to the primary purpose of identifying lung cancer. Most IFs on LCS are benign and clinically insignificant but are being increasingly recognized, and some require urgent referral for further diagnostic workup. Other findings are expected and are known as smoking-related comorbidities, including COPD, cardiovascular disease, emphysema, and interstitial lung disease, and their diagnosis can have a significant impact on patient prognosis. The purpose of this pictorial essay is to illustrate the most common IFs on LCS, organized by organ. We will discuss the current literature on IFs on LCS, focusing on their prevalence, appropriate communication, and triggering of clinical pathway systems.


RESUMO Os programas de rastreamento de câncer de pulmão (RCP) estão aumentando em todo o mundo. Achados incidentais (AI) no RCP são achados de TC de baixa dose sem relação com o propósito principal de identificar câncer de pulmão. A maioria dos AI no RCP é benigna e clinicamente insignificante, mas os AI estão sendo cada vez mais reconhecidos, e alguns requerem encaminhamento urgente para investigação diagnóstica adicional. Outros achados são esperados e conhecidos como comorbidades relacionadas ao tabagismo, tais como DPOC, doença cardiovascular, enfisema e doença pulmonar intersticial, e seu diagnóstico pode ter um impacto significativo no prognóstico do paciente. O propósito deste ensaio pictórico é ilustrar os AI mais comuns no RCP, organizados por órgão/sistema. Discutiremos a literatura atual a respeito de AI no RCP, com foco em sua prevalência, comunicação apropriada e desencadeamento de procedimentos clínicos.

ABSTRACT

RESUMO Objetivo Avaliar o impacto da triagem de retinopatia diabética de paciente diabéticos realizada com retinografia colorida. Métodos Estudo retrospectivo, de caráter descritivo, avaliando laudos de retinografias realizadas desde a implementação do protocolo da triagem de retinopatia diabética de paciente diabéticos acompanhados no Ambulatório de Endocrinologia de um hospital terciário do Sistema Único de Saúde, de maio de 2018 até maio de 2020. Resultados Realizaram retinografia 727 pacientes diabéticos, que tinham entre 14 e 91 anos, sendo a maioria com 60 anos ou mais (53,2%), do sexo feminino (68%) e brancos (87,6%). Não apresentavam retinopatia diabética 467 (64,2%) pacientes, 125 (17,2%) tinham retinopatia diabética não proliferativa, 37 (5,1%) retinopatia diabética não proliferativa grave e/ou suspeita de edema macular, 65 (8,9%) retinopatia diabética proliferativa, 21 (2,9%) suspeita de outras patologias, e as imagens de 12 (1,7%) pacientes eram insatisfatórias. Foram considerados de alto risco (aqueles com retinopatia diabética não proliferativa grave e/ou edema macular, retinopatia diabética proliferativa ou imagem insatisfatória) 114 (15,68%) pacientes. Conclusão O rastreio de retinopatia diabética com retinografia colorida possibilitou a detecção de pacientes diabéticos de alto risco que necessitavam atendimento com brevidade, permitindo o acesso deles à consulta oftalmológica e diminuindo a morbidade da doença relacionada ao tratamento tardio. Os demais foram encaminhados à Atenção Primária para regulamentação, por meio do Sistema de Regulação.


ABSTRACT Objective To evaluate the impact of diabetic retinopathy (DR) screening using color retinography in diabetic patients. Methods Retrospective descriptive study, evaluating reports of all retinographs performed since the implementation of the protocol for screening for diabetic retinopathy in diabetic patients followed up at the endocrinology outpatient clinic of a tertiary hospital of the Unified Health System, from May 2018 to May 2020. Results 727 diabetic with age range from 14 to 91 years old, the majority being 60 years old or older (53.2%), female (68%) and white (87.6%), patients underwent retinography. Of the patients, 467 (64.2%) did not have DR, 125 (17.2%) had non-proliferative DR, 37 (5.1%) had severe non-proliferative DR and/or suspected macular edema, 65 (8.9%) had proliferative DR, 21 (2.9%) had suspicion signs of other pathologies and 12 (1.7%) had unsatisfactory images. A total of 114 (15.68%) patients were considered at high risk (those with severe non-proliferative NP and/or EM, proliferative DR or poor image) and were referred for comprehensive ophthalmic evaluation. Conclusion The screening of RD with color retinography enabled the detection of high-risk diabetic patients who needed assistance sooner and enabled their access to ophthalmologic consultation, which decreased disease morbidity. The others were referred to primary care for regulation through the Regulation System (SISREG).

Background: Timely eye fundus examinations are essential to prevent the consequences of retinopathy among patients with type 2 diabetes mellitus. Aim: To assess the coverage rate (CR) of eye fundus examination in the Chilean diabetic population, between 2011 and 2019. Material and Methods: Analysis of monthly statistical summaries of the Cardiovascular Health Program published online by the Chilean Ministry of Health. The number of patients aged 15 years or more with a diagnosis of Type 2 diabetes mellitus and the number of those who had an eye fundus examination within the same year, were obtained. Data was analyzed at a national and regional level. Results: The national eye fundus examination CR was 19.1% in 2011. In 2016, the figures became significantly higher, reaching 32.5%. In 2019, the highest value of 36.5% was recoded although not significantly different from 2016. The highest average annual CR was observed in Ñuble Health Service (49.5%), and the lowest in Central Metropolitan Service (15%). The highest CR positive absolute and relative variation between 2011 and 2019 was observed in Viña del Mar Quillota Health Service (38.9 and 489% respectively), and the lowest negative variation was observed in Araucania Norte Health Service (-8.42 and -24.21% respectively). Conclusions: There is a low eye fundus examination CR in Chile, with important differences between regional health services.

Esses conceitos são parte integrante das recomendações do Protocolo Clínico e Diretrizes Terapêuticas para Atenção Integral às Pessoas com Infecções Sexualmente Transmissíveis (IST) publicado pelo Ministério da Saúde do Brasil em 2020. O artigo propõe uma abordagem abrangente da sexualidade para promoção da saúde e apresenta aspectos importantes do processo de comunicação, que deve ocorrer de forma clara, sem preconceitos ou juízos de valor, com foco na saúde sexual e reprodutiva. Destacam-se pontos relevantes acerca do exercício da sexualidade em fases específicas da vida, recomendando avaliação dos riscos e vulnerabilidades, bem como o rastreamento de IST e o uso de preservativos. Dessa maneira, é possível contribuir para que as pessoas possam exercer sua sexualidade de forma plena, responsável e segura.

Resumo Objetivo Rastrear o risco de sarcopenia em indivíduos hospitalizados por meio dos instrumentos SARC-F e SARC-Calf e verificar a associação entre o risco de sarcopenia com as variáveis sociodemográficas, clínicas e as variáveis que compõem o fenótipo de sarcopenia. Métodos Trata-se de um estudo transversal. Características sociodemográficas, clínicas e todas as variáveis que compõem o fenótipo de sarcopenia (força de prensão palmar, massa muscular e velocidade de marcha) foram investigadas. Para o rastreamento e diagnóstico da sarcopenia adotou-se o algoritmo e critérios propostos pelo European Working Group on Sarcopenia in Older People (EWGSOP2). Resultados Participaram 90 indivíduos. A maioria encontrava-se sem sinais sugestivos de sarcopenia, tanto pelo SARC-F (58,9%) quanto pelo SARC-Calf (68,9%), com força de preensão palmar (FPP) (28,59±9,21;26,74±10,60) e índice de massa muscular esquelética apendicular (IMMEA) (9,31±1,78;9,58±1,62) normais e com baixa velocidade de marcha (VM) (0,69±0,26; 0,68±0,44), respectivamente. O SARC-F apresentou associação significativa com as variáveis sexo (p=0,032), FPP (p<0,001), VM (p=0,001) e sarcopenia (p<0,001). Quando da adição da circunferência da panturrilha (CP), foi encontrado associação com as variáveis grupo etário (p=0,029), atividade laboral (p=0,008), FPP (p<0,001), IMMEA (p=0,033), VM (p=0,019) e sarcopenia (p<0,001). Conclusão O risco de sarcopenia foi observado em aproximadamente um terço dos pacientes avaliados. Sugere-se o uso rotineiro nos hospitais da ferramenta de rastreamento de sarcopenia SARC-Calf, uma vez que apresentou associação com os três fatores preditivos da sarcopenia, além de ser um instrumento de aplicação ágil, baixo custo e não invasivo. A investigação do diagnóstico da sarcopenia deve ser encorajada na prática clínica.


Abstract Objective To screen the risk of sarcopenia in hospitalized individuals using the SARC-F and SARC-Calf instruments and verify the association between the risk of sarcopenia with the sociodemographic and clinical variables and those that make up the sarcopenia phenotype. Methods This is a cross-sectional study. Sociodemographic, clinical characteristics, and all variables (handgrip strength, muscle mass and gait speed) that construct the sarcopenia phenotype were investigated. For the screening and diagnosis of sarcopenia, the algorithm, and criteria proposed by the European Working Group on Sarcopenia in Older People (EWGSOP2). Results A total of 90 individuals participated. Most were without risk of sarcopenia, both by SARC-F (58.9%) and by SARC-Calf (68.9%), with normal handgrip strength (HGS) (28.6±9.2; 26.7±10.6) and appendicular skeletal muscle mass index (ASMI) (9.3±1.78; 9.6±1.6) and with low gait speed (GS) (0.69±0.26; 0.68±0.4), respectively. SARC-F showed a significant association with the variables gender (p=0.032), HGS (p<0.001), GS (p=0.001) and sarcopenia (p<0.001). When adding the calf circumference (CC), an association was found with the variables age group (p=0.029), work activity (p=0.008), HGS (p<0.001), ASMI (p=0.033), GS (p=0.019) and the sarcopenia (p<0.001). Conclusion The risk of sarcopenia was observed in approximately one-third of the evaluated patients. It is suggested the routine use in hospitals of the sarcopenia screening tool SARC-Calf, since it was associated with the three predictive factors of sarcopenia, in addition,it is an instrument of agile application, low cost and non-invasive. When a possible, investigation of the diagnosis of sarcopenia should be encouraged in clinical practice.

ABSTRACT

Background: Cervical cancer is one of the most prevalent cancers in the world among women which its early diagnosis plays an important role in the prognosis. There are many factors that contribute to the participation in the screening programs, most notably the level of knowledge and attitude of people towards cancer. Understanding the level of these factors in the female population and its association with participation in screening programs is important. Therefore, the aim of this study was to determine the knowledge, attitude and practice of women in the north of Iran to compare these factors between two groups with different baseline knowledge.Methods: This cross-sectional study was carried out among female medical clients and healthcare staff in a healthcare center in the north of Iran. All the eligible patients were interviewed and were asked to fill a questionnaire containing the demographic data, knowledge, attitude, and action about cervical cancer and screening. The data were analyzed by SPSS v24.Results: A total of 283 females entered our study of which 156(55.1%) were clients of the healthcare center and 127(44.9%) were non-physician healthcare staff. Ninety (60.8%) of clients and 39(56.5%) of the staff have performed pap smear at least once (p=0.556). The levels of knowledge and attitude were significantly lower in the clients (p < 0.001 and p=0.003, respectively). There were no significant differences regarding the level of knowledge and attitude between those who performed pap smear and those who did not (0.621 and 0.788, respectively).Conclusions: Increasing awareness, especially improving attitudes in the female population, should be the focus of the health care system to encourage more women to participate in screening programs.

RESUMEN Objetivo Estimar la cobertura de mamografías de tamización en Colombia durante el año 2014; así como el rendimiento en horas a emplear por equipo y el número de equipos requeridos, para alcanzar la meta de cobertura del 70% de mamografía de tamización antes del año 2021. Materiales y Métodos Se realizó un estudio descriptivo para la caracterización de equipos de mamografía existentes en el país. Para ello, se requirió la información del Registro Especial de Prestadores de Salud (REPS) y la información de licencias de funcionamiento de equipos de mamografía tramitados ante los entes territoriales; la información fue procesada, validada y analizada a través de tablas de distribución de frecuencias, en el programa Microsoft Office Excel®. Resultados La estimación de cobertura de mamografía de tamización para el país, con base en el censo de mamógrafos, es de 27.6%; para alcanzar la meta de cobertura de mamografías de tamización del 70% a nivel país, un mamógrafo requiere emplear semanalmente 38.2 horas para tamización, el 56,3% de su tiempo total para mamografías de tamización y adquirir aproximadamente 69 equipos adicionales a los censados en el año 2014. Se muestran marcadas diferencias en estos datos a nivel departamental y regional. Conclusión A nivel país, la estimación de cobertura de mamografía de tamización no es óptima; y se requiere mejorar el rendimiento de los equipos de mamografía, así como adquirir equipos adicionales para algunas regiones, acompañado de estrategias programáticas eficaces en detección temprana de cáncer de mama.(AU)


ABSTRACT Objective Estimate the coverage of screening mammograms in Colombia during 2014, as well as the performance in hours of equipment by the team and the number of teams required, to reach the goal of 70% coverage of screening mammography before the year 2021. Materials and Methods A descriptive census study was carried out to characterize existing mammography equipment in the country. To this end, information was required from the Special Registry of Health Providers (REPS) and the operating licenses of mammography equipment processed before the territorial entities; the information was processed, validated and analyzed through frequency distribution tables in the Microsoft Office Excel® program. Results The estimation of screening mammography coverage for the country based on the census of mammographers is 27.6%; To reach the screening goal of screening mammography of 70% at the country level, a mammograph requires weekly 38.2 hours for screening, use 56.3% of total time for screening mammograms and acquire approximately 69 additional equipment to those screened in 2014. There are marked differences in these data at the department and regional level. Conclusion At the country level, the estimation of screening mammography coverage is not good, it is necessary to improve the performance of mammography equipment, as well as acquire additional equipment for some regions; accompanied by effective programmatic strategies in early detection of breast cancer.(AU)
